To initiate the process of self-referral or command referral, a DA Form 8003 must be completed and turned in to the Dept. of Behavioral Health, Building 18. Once the form has been completed, the Soldier will see a provider at Camp Atterbury and the Soldier's information and DA Form 8003 will be sent to Ft. Knox. A Soldier will then be assigned an ASAP provider at Ft. Knox and will also be provided with behavioral health resources within the community sounding Camp Atterbury, follow up with a Licensed Clinical Social Worker at Camp Atterbury, and report all progress and needs to their ASAP provider at Ft. Knox. Please note: for the initial appointment at Fort Knox, the soldier will need to be escorted by another service member of higher rank (E-5 or above) and that escort will need to remain available until a final disposition has been made. If any transportation to an outside facility is necessary, the escort is to provide it. ASAP personnel cannot provide this for the service member being treated. |
